Our Community Service offers care in the patients home. This is supportive care and helps patients with difficult symptom and pain control problems, provides emotional support including support to families and carers.
The community care we provide is primarily given by our Specialist St Clare Hospice Macmillan Nurses. Our medical team, physiotherapist, occupational therapist and chaplain will also visit patients in their own home.

Equipment Loan Service
We provide an equipment loan service offering riser recliner chairs and nebulizers. We have close working relationships with the Independent Living Service of West Essex, and the Red Cross who provide other vital equipment and can deliver directly to patients own homes.
We offer an advice line which operates 24/7. If you are a patient, carer, friend or a health professional, if we can help in any way, we will. We can offer advice on symptom control, services available, referral process and bereavement support to name but a few. Your call will be dealt with by our experienced nursing team.
Support for family and friends is vital. We can support you through our carers group, informally, or through our Complementary Therapy service. We recognise caring can be exhausting – we are here to help.
It is often comforting for patients to know that their family and friends will be cared for after they die. We offer a full bereavement service for everyone who has been cared for, or is known by the St Clare Hospice Team.
